Office policies and procedures for our patients
Thank you for choosing Anthem Primary Care! We are pleased that you chose us!
Our staff at Anthem Primary Care strives to exceed your expectations in care and service, so that your visit is as comfortable and stress free as possible. We aim to provide quality of medical care focusing on your needs. We prioritize your health and have implemented these policies and procedures to provide you care in timely manner.
Office Hours:
Our office is open Monday-Friday:9:00AM-5:00PM, Sat: 9:00AM- 12:00 PM, you can reach us at 719-309-2583, this is our patient line.
Appointments can be cancelled 24 hrs before the appointment time:
We have a charge of $50 per call for after hour care: you can call us at after hour care line and you will be prompted to get to the physician.
No show for office visit: there is a charge of $50
No show for procedure visit: there is a charge of $100
Please check with the office for the enrollment charges.
If you are Anthem Direct Patient you can reach the provider by texting or email or phone any time you need them, free of charge.
If you need an appointment, prescription refill or tests results, please call during regular business hours.
We are NOT a walk-in clinic. We need you to make appointments with the provider. You can conveniently check their schedule and choose what fits your availability. Our providers have both online and in person visit options available for your convenience.
Some special services require first appointment as in person visit to the office. If you make an online appointment for such a visit then we will call you to schedule an in person visit.

Anthem Primary Care is committed to providing quality care and services to our patients. We encourage patients to schedule appointments in advance of follow up due dates:

We strive to give all patients the time they require and we schedule appointments appropriately but sometimes if there is an emergency, we might have to delay or reschedule your appointment so we request your patience and understanding, should that happen to in your case.
To maintain the quality of care, we do not treat patients we have not seen and are not enrolled in our clinic (that is. We will not call-in prescriptions or other medical advice. We need you to make appointment with us)
Follow up visit may be required to be scheduled after testing is completed to review the results together to determine an next steps in your healthcare.
We encourage you to schedule appointments for:
- Preventative health Visits
- Physical check ups
- Pap exams
- Chronic Medical condition
- Prescription renewal
- Sick visits
CANCELLATION of an Appointment:
Please call us to notify that you cannot attend your appointment.
If you are unable to attend an appointment, please be courteous and inform Anthem Primary Care, there might be a patient who needs medical attention and needs an appointment, so please be respectful of the medical needs of our patient community. To serve best to the needs of our patients we request you to inform us so your canceled appointment can be allocated to someone who is need of treatment.
Appointments must be cancelled 24 hrs before the appointment time to avoid any fees which will not be covered by your insurance.
NO SHOW POLICY:
No show is a person who misses appointment without canceling 24hrs before their appointment. It causes inconvenience to the patients who need medical attention and care in a timely manner. It also creates waste of time and resources behind preparation of your appointment.
No show will be recorded on your medical chart and a letter will be sent to you alerting the fact that you were a no show for your appointment and an administrative fee/ noshow fee of following denominations will be billed to your account:
No show for office visit: there is a charge of $50
No show for procedure visit: there is a charge of $100
3 or more no shows in a year will lead to temporary suspension of services.
In order to reestablish as a patient, you will be required to meet with your Primary Care Physician within 30 days of the third no show letter to evaluate your circumstances. In the event you do not respond and/or schedule an appointment within 30 days, we will consider your patient status as terminated. **Please note that No-Show charges are patient responsibility and will not be billed to your insurance company.
DISRUPTING PATIENTS
For the safety of our staff and other patients we will not accept any means of disruption. We strictly follow NON VIOLENCE and any sort of violence or disruption will lead to reporting to the police and perhaps discontinuation of the services.
INSURANCE:
Anthem Primary Care accepts most of the major plans. You will have to reach out to your insurance company to confirm your eligibility. It is your responsibility to inform our office of any changes in insurance coverage, else it could lead delay or denial of payment.
Co-Pays are patient responsibility and has to be paid before your service. Depending on your insurance contract, if service is not covered by your insurance then we will bill it to your account.
PAYMENTS:
Anthem Primary Care accepts all major credit cards including Visa, Mastercard, Discover and Amex and also Personal Checks and Cash.
Balances left unpaid and without further arrangements within 90 days may result in dismissal from the practice, balance sent to collections, and/or notification to your insurance plan. Any returned checks will be charged a $25 fee in addition to any bank fees.
PRESCRIPTION: Prescription refills and Pharmacy information:
Anthem Primary Care will send your prescription to the pharmacy you have given us. Patient can inform us or update pharmacy information at the time of visit and check in.
Please review all your medications and talk to your physician and pharmacist if you have any questions about your medications.
Antibiotics and pain medications will not necessarily be filled or refilled over the phone. Your healthcare provider will decide whether it important for you to make an appointment to reevaluate your condition. In that case you can make an appointment using our convenient online scheduling or call us we can help you find one.
